GENERAL INFORMATION

Change of circumstances
Please help us to keep your personal details up to date by notifying us immediately of any changes e.g. change of name, address, telephone number etc. If you move outside our practice area, you should registered with a new doctor as soon as possible so that your medical records can be sent on.

Suggestions and Complaints
The partners and staff are keen to provide patients with the best possible care. Therefore, if any patient has a suggestion for improving our service or a constructive criticism of the service offered, please do discuss this with our Practice Manager. Mistakes and misunderstandings can happen from time to time and our aim is to resolve these as quickly as possible.

This practice fully supports putting our patients' needs first and offering our services in a very positive way. A copy of the practice's charter and complaints' procedure is displayed in the surgery.

The Practice is fully registered under the Data Protection Act. This Practice recognises the need for confidentiality and all staff realise the importance of total discretion on all matters at all times.

Confidentiality
The confidentiality of your personal information is as important to us as it is to you. Your medical records are, out of necessity, accessible to a small number clerical and administrative staff within the practice. They are all trained to keep your personal information strictly confidential. No other persons may access your records without your expressed permission. Further information on this topic is obtainable from our Practice Manager.

Abusive or violent behaviour
Abusive or violent behaviour towards staff, or any other person, on our premises is not tolerated by this Practice. Patients who are abusive may be required to register at another Practice. In line with all NHS organisations, we operate a zero-tolerance policy to violent behaviour and our staff will not hesitate to call the police in such circumstances.

Facilities for the disabled There are adequate parking, access and toilet facilities for disabled people. The staff will be pleased to help, if needed.
