PRESCRIPTIONS AND APPOINTMENTS ON-LINE

READ THIS FIRST

To book an appointment, or order a prescription, on-line you will need a PIN number and Access ID to create an account on our system. These are supplied by the practice and are individual to each patient (i.e. are not transferable). To obtain this you will need to visit the surgery reception and MUST bring with you some photo-ID (i.e. passport or new-style driving license).

VERY, VERY IMPORTANT: When booking an appointment on-line you will be presented with a choice of six doctors (the six Partners of this practice). Three work at Chalfont St Giles and three at Chalfont St Peter. It is VITAL therefore that you select a doctor at the surgery where you are registered.
